Yorhealth, who are we?
At Yorhealth we provide bespoke nurse-led care for children and young adults with complex medical care needs throughout Yorkshire. We support children and families in their own home to care for their children. We also support children in educational settings to continue with their education, and to achieve their goals and aspirations. By working closely with commissioners and in partnership with families we ensure the safe delivery of care that enables children and young people to live their lives at home.
At Yorhealth we have developed a handpicked team of staff, bespoke to each family that we work with, operating 24 hours a day, 7 days per week. Our care is overseen by our Senior Clinical Nurse Specialist Team and supported by trained Children’s Clinical Complex Support Workers.
We are a centre of excellence for children’s home care ventilation and provide bespoke care for the following;
The conditions, health care and wellbeing need’s that we support:
- Oxygen management and saturation monitoring
- Tracheostomy care including non-invasive and invasive ventilation
- Enteral feeding support including gastrostomy, peg and jejunostomy feeding
- Medication management including administration of emergency meds.
- Moving and Handling
- Orthotic management
- Physiotherapy
- Work alongside occupational therapists, SALT therapists and sensory impairment teams.
- Seizure and dystonia management.
- General personal care, with high levels of dignity
- Supporting education and emotional wellbeing.
